If you wish to change your primary care provider, please call the member services number on the back of your insurance card.
Call your primary care provider's office to schedule your appointment. If you're not sure who your primary care provider is, call the member services number on the back of your insurance card. They'll be able to help you.
- Bring your prescription bottles or a list of your medicines. That includes herbs and vitamins. This list should have the name of the medicine, the amount and the provider who ordered it for you.
- Bring any previous, relevant medical records you may have.
- Bring your health plan ID card and picture ID (for example, your driver's license).
First, you'll want to schedule an appointment with your primary care provider. Talk to them about any worries or health problems you may have. Based on your visit, they'll refer you to a specialist if needed and give you next steps.
