If you wish to change the primary care provider you’re empaneled to, please call the member services number on the back of your insurance card.

Call your primary care provider’s office to schedule your appointment. If you’re not sure who your primary care provider is, call the member services number on the back of your insurance card and they can assist you.

  • Bring your prescription bottles or a list of your medications, including herbs and vitamins. This list should include the name of the medication, dosage and the provider who prescribed it to you.
  • Bring any previous, relevant medical records you may have.
  • Health insurance ID card and picture ID (for example, your driver’s license).

First, you’ll want to schedule an appointment with your primary care provider to discuss any concerns or conditions you have. Based on your visit with your primary care provider, they’ll refer you to a specialist if needed and provide next steps.
