Your Annual Wellness Screening
Preparing for your health journey
Annual wellness visits are an important part of keeping you on the path to good health and are at no cost to you. Remember to bring the following to your appointment:
- A list of all of your medications.
- Your past medical records. If you do not have your medical records, please bring your previous provider's information so we can request them for you.
- A photo ID and your new insurance card.
What to expect at your appointment
During your exam, you’ll spend extended time with your health care team, giving you the opportunity to ask questions. This helps your provider get to know you and your medical history. As part of the visit, you may receive tests and screenings based on your individual needs. You’ll also:
- Discuss and plan management of long-term conditions
- Review your medications
- Ask any questions about your overall physical and mental health
